Bureaucracy Risking Patient's Life

Dear President Obama: I am a 65 year Psychiatrist Board Certified in Psychiatry with Added Qualifications in Addiction Psychiatry. I am near the end of my career so I am not writing this for personal gain. The system of prior authorization for prescription medications has become an obstacle course that is putting the life of patients at risk. Particularly with doctors that are too busy to do it. I have a patient with chronic pain as a result of multiple injuries sustained in his military career as a Navy Seal in Black OPS. He served for approximately 18 years. He was nominated and accepted by congress for official membership in the American Legion for gratitude for his war-time service. He was treated with opiate pain medications and became addicted to them and he was referred to me by his pain doctor in 2004. I have been treating him with Suboxone since then and he was stable. He got Medicaid over 1 year ago and suboxone was approved until 6 months ago. Then it was denied. I have sent multiple prior authorization requests that have taken many hours of my time. After several denials which didn’t make any sense. I Finally I got a denial stating: "Medicaid policy supports Suboxone for rapid taper or slow taper. Indefinite therapy is not considered for reimbursement. For recipients with co morbid chronic pain uncontrolled by non -controlled substance treatment interventions, Methadone Clinic is a Medicaid treatment option." Since they denied paying for the medication he has been suffering from pain and has been prescribed again narcotic pain medications by his pain doctor. The suggestion of a methadone clinic in this patient is absurd. Methadone is an full opiate agonist. There are over a thousand deaths every year of chronic pain patients with addiction that are prescribed Methadone. At this point Medicaid is putting this patient’s life at risk and there seems to be nothing else I can do. Respectfully yours, Bernardo Savariego MD

Abuse of Our Healthcare System

Dear mr president. My  name is Renee i am a nurse and i have decided to write this letter because i am really tired of the abuse in our healthcare when i have to pay so much for health insurance. The patients who have medicare espiciallly the young ones  abuse it more.The day that has driven me to write this letter is a patient at my hospital wanted to go out side for a smoke she is on a unit that monitors her heart so she is very sick and cannot go out side the patient decided to leave the hospital AGAINST MEDICAL ADVISE even though she could die ,she left any way and said medicare is paying and i will just go back to the ER when i am done .That has happened so many times that my co-workers and i are fed up of this misuse thes patients give us the hardest time and appreciate u less because you care about their health. I think there should be a clause that prohibits this from happening,there should be consequences other than sending them a bill they are not going to pay. One of my co-worker just told me to tell you that may it would not be a bad idea to get some of our opinions ,people who work on the floor.

Cobra for the unemployed

Mr. President, I am finding myself in a desperate situation and have to wonder how many others are experiencing the same as me.  I am HIV positive and have had full-blown AIDS 3 times in the last 15 years.  Each time it was because I was off of my medications. I was  laid off in November and am unable to afford the Cobra payment of $511 per month.  My medications are about $3000.00 per month.  I am seeking employment, but cannot afford either the Cobra or the full price of the medications.  I am receiving $300 per week from unemployment insurance which barely covers my living expenses, let alone the Cobra payments. I am afraid that I will be forced to go without my meds which will most certainly mean death to me. I'm sure there are many who are in my position.  HIV assistance programs in Virginia are not accepting any new applicants.  I am not sure where to turn. Sincerely,   Janet

Pay back unemployment benefit?

My daughter was let go from her place of employment unjustly and was able to receive unemployement benefits.  She is a single mother who is trying to better herself by attending Nursing school all the while looking for a way to support her family.  She is an excellent mother and maintains a 4.0 GPA.  After receiving an extension on her unemployment benefits, the state decided that she had to give back all of the money.  I know it must be expensive to support all of these people with extensions, but what good is giving the benefit in one hand and taking it away with the other.  She is not working yet and given the stress of being a single mother and student, this is just one more thing for her to worry about.  I don't know what I'd like for you to do to remedy this, but I understand that she is not the only one that this is happening to, there are many others.  Please look into this sad situation that is terrorizing many people in the state of Indiana.   Thank you for your time, Grace

A letter to Jackson Health System Personnel, Patients, Healthcare Organizations and MediaI used to be a high-risk pregnant patient of Obstetrics and Gynecology at the Ambulatory Care Center of Jackson Memorial Hospital. By the end of the second trimester, I was so frustrated and distressed by their poor service that I decided to find another prenatal care provider to gain back my peace of mind. Since I started receiving a decent prenatal care in the new place, I have felt happier and more focused on the joys of pregnancy. However, I am still trying to clear up a medical billing claim, which has not been attended by the Customer Service Department of the Jackson Health System. I could give details about all the inconveniences or abuse that I perceived and experienced myself at the Ambulatory Care Center and other facilities of the Jackson Health System; but I want to be concise so I will point out just a few.· Humiliating comments about patients by some healthcare personnel· Lack of serviceable attitude and courteous manners· Inattentive Customer Service Department· Negligent and disrespectful medical staff· Failure to comply with Patient Privacy, by discussing medical information in waiting area· Lost medical records and lab tests resultsUnfortunately, there are many people under these circumstances or even worst situations. Some of them do not know their rights. For this reason, I am spreading the word by all means possible, trying to get someone’s attention toward the problematic situations that patients go through in this important healthcare institution. I also want to have my problem solved, since I cannot –I do not have to– pay a pricey medical bill for services that I did not receive.If you are a patient, make sure you know your rights and procedures to report any complaints. You may call the Agency for Health Care Administration at 1(888) 419-3456.Remember patients deserve to receive their medical treatment with dignity. I hope this letter gets into the right hands! President Obama, The problem with our public school system is not just with the dropout rate. The system itself is truly broken and in my opinion corrupt. The bad teachers even know it and take advantage of it fully. For over 20 years I have watched my own children, relatives, friends and fellow employees children in the system. I have seen the really smart children drop out of high school simply because they could see through the system. They lost respect for the system and many of the teachers. The other children that did manage to graduate and even made "good" grades can't spell, read well or do simple math well. They are the ones having the hardest times. The system just passed them through from elementry through high school without really teaching them. Parents got good report cards and didn't realize the problems they had until later. The schools get paid according to how many students they have and how many graduate not how well they are really teaching them. In the past it has made me so angry I wanted to sue them for what they did to my children. And yes I did do the little homework that they brought home and tried to help them as much as I could. My daughter wasnt even allowed to bring her text books home with her. In my job I was responsible for hiring young guys to work in a banquet facility. Most of them had just graduated but couldnt fill out the simplest employment application on their own. If you want to see the proof get someone to go out on the streets Pick 50 people between 19-25 that graduated from a public school, not in college, and test them. You will be surprised at how many are lacking the very basics that they need.Mr Obama when people are telling you how to fix our education problems please be very careful with how it is done and keep my letter in mind. It is hard to get young people to admit that they know they don't have what they need but if they do have what they need you will see a lot more of them with the confidence to try to go to college in this country. We have been failing our children for years and now it is going to hurt our country if it is not repaired.And just an FYI it is not only the poor, inner city, black and Mexican children. My family is in the middle class,white,very nice Dallas, Texas area. It is all of our children that are being cheated.I do believe you are doing a great job up against amazing odds and sincerely thank you for trying so hard.

Education Reform

Mr. Obama,You talk a lot of reforming education which is greatly needed in this country. There are tons of teachers that need to be removed from teaching and expected to do something else. This is true, how to do that is questionable. There are probably many different ways to get this done.The one thing that you do not talk about that is a big picture of education and the success of students in the classroom is there family. If you want to reform education you will have to do something about the family and the home. Education is not a priority in many homes and that leads to less then success in the classroom. If a student comes home with a bad grade and the parent does not care or isnt even there then why should the child care. Kids have a difficult time with self motivation. They need to be nurtured and know that someone cares about them. There is a lot going on in a classroom that cannot be judged by testing measures. The students have a lot to deal with and unfortunately education is sometimes last on there plate. Students need to be loved, cared for, and given attention both at home and at school in order to feel sucessful. I understand that you cannot "control" what happens at home to every child because by your standards they are taken care of. Imagine coming home at 8 years old and your mom or dad is still at work. Your little brother got off the bus with you and it is just the two of you at home. You watch TV until mom comes home in about two hours. She is exhausted and still needs to fix dinner and get you ready for bed. You have homework that needs her help but she doesnt have time or doesnt know herself how to do it? Who is going to help this child? Who is going to show him that someone cares about whether he is successful or not? His parents are doing what they can to help him but in some ways they are faliing. They are good parents, they work to feed him and clothe him but it just isnt enough. How can you blame teachers solely for the failing education system? Money is a problem but it is not the only problem! Home life, poverty, poor relationships, emotions, and lack of character are among some that create bigger issues then money ever could.If you look at the home life in the countries that you mentioned, it is remarkably different from ours here in America. I am not sure how you fix this but I didnt want you to leave it off.

Healthcare Struggle with Cancer

Dear Mr. President Obama  

My name is Amanda Collins and I am disgusted with the healthcare situation, my brother Brett had stage 3 cancers and it’s not the fact he didn’t have coverage, but it was a limited policy covering only $3000.00. by Brett being 21, he didn’t know and thought if you have insurance you should be covered, Brett’s first visit to the emergency room exhausted his benefits, leaving him with no insurance, now his treatment has been basically been put on the back burner, he did receive chemo for 5 day a week for 2 months, had tumors removed, but now needs other treatment.  He is on oxygen and has had 4 blot clots in his lungs, he also needs another surgery.  

My problem is why isn’t a person who is diagnosed with cancer given state funded insurance right away?? When we give children who are born in this country illegal insurance, we also give them food stamps?? I feel as though a person with this illness should be given attention as soon as the problem is diagnosed and not put on a waiting list. Some people might not have 90-120 days.  

My mother has been his voice, but what about the people who have no one, no insurance, nothing, who is there voice, my brother is 21, has his whole life on hold, and living in the united states, it makes me sick that he has been a tax payer, working two jobs, and now he cant work, cant get benefits and now has disease that can destroy a persons being.   My brother is my hero, as he has remained in good spirits and takes everyday as another day to get better, hasn’t even said “why me”, he never feels sorry for himself. So you tell me why people who have cancer running through there body and may loose the fight shouldn’t be given state insurance right away, why make getting state insurance another thing to fight for??   My mother has made a box of all the bills that come in on a daily basic, the amount of money that he s owe’s is rediculis, why should he have to worry about this??? I don’t get it. Give me a reason.   

This weekend he was in so much pain and didn’t want to tell anyone cause he knew it would be another visit to the er, which means another bill, my mother made him go, as she saw how much pain he was in, when he got to the er, he was given oxygen, and had ct scans done, which showed that his chest was filled with fluid, the doctors explained it as “he was drowning in his own body”. They had to cut him and drain all the fluid.   My mother sent me a picture of my brother thru the phone and he had a smile and his thumb up. How can anyone deny my brother?   In my eyes he is still a little boy, with his whole life in front of him, he was planning to go to college, now everything has been put on hold just like his medical care.   I wish I could do something instead of writing this letter, but I don’t know what to do, I also have worked my whole life two jobs, one being in the medical field, and I get so mad when I see illegal people who haven’t paid anything in to the system get the benefits that we have worked for what gives them the rights over my brother???   I hope this letter finds someone to help people who need help really get it. The cancer society can only do so much.  I thank the hospital and the doctors that have helped my brother I just wish it could be easier.   If some one could help me or guide me in the right direction to help my brother it would be greatly appreciated.

DEAR MR. IBAMA I AM SO DISAPPOINTED IN OUR SYSTEM AND THE WHY OUR GOVERNMENT WANTS TO DICTATE OUR LIVES THAT I'M FOR CHANGE IN A BIG WAY.. EVERYONE YOU TALK TO HAS HAD A BAD EXPERIENCE WITH OUR SYSTEM OF GOVERNMENT... I HAVE WORKED MY HOLE LIFE AND SO HAS MY HUSBAND UNTIL HE GOT SICK AND I SEE HOW SSI TREATS PEOPLE LIKE DIRT AND MAKE YOU WAIT AND WAIT I MEAN YEARS NOT MONTHS.I WOULD LIKE TO SEE THEM LIVE WITH OUT ANY MONEY AND TELL ME HOW IT IS...MY HUSBAND WORKED AND DESERVE MORE THEN HOW THEY TREATED HIM ( LIKE A DOG) THAT OUR GOVERNMENT AND YES I AM NOT HAPPY WITH THEM...PEOPLE TALK AND YOU HERE THE STORIES WELL WE ARE TIRED OF PROMISES THAT DON'T COME TRUE..OUR STATE REPRESENTATIVE DON'T DO ANY THING BUT BRING IN MINAMUM JOB WAGE WELL LET PUT THEM ON A LOWER PAY WAGE...I AM SO SICK OF THIS GOVERNMENT THAT I INTENT TO VOTE EVERYONE OF THEM OUT. THAT HAVE NOT DONE ONE THING FOR THE PEOPLE ...WHAT ABOUT US WE PAY TAXES ....STRICKLAND HAS TAXED US WE PAY HIGHEST TAX IN US.. AND WE HAVE NOTHING HERE...TIM RYAN HAS DONE NOTHING BUT GIVE MONEY TO YSU.. WELL ENOUGH IS ENOUGH...WE NEED SOMEONE WHO IS GOING TO CHANGE OUR WHOLE SYSTEM ....NOT JUST SAY LOOK AT ME....WHAT IS GOOD FOR US SHOULD BE GOOD FOR THEM.....SO LET CHANGE THEIR PAY AND GROUND THE PLANE .....GOVERNMENT HAS NOT CUT THEIR POCKETS ...WE HAVE..
